The diffusers are an adaptation of a set up I found on the web. There's actually two diffusers, one a stofen type to fit on the flash heads, the other is the type of diffuser you can get to go over camera built in flash heads.
The two are bolted together. Works quite well. Those shots were done on ETTL flash setting.
To make it clearer here's one of them:
The other change is the flexible head support. That works very well, easily adjustable to any angle.
And here's the standard MT-24:
