Apparently 2-spots can have no spots or up to 16 spots! I can't find many pics showing variations (other than red on black), in particular those with more than 2 spots on the standard red elytra form. Those with more than two spots I have found have had only small black spots.
This I found inside, size ~4mm. At first I thought the extra large spot was damage as I have seen the occasional damaged 7-spots, it was only when I downloaded the pics I saw it was a large spot on one elytra only!
Has anyone seen one similar?

your first thought was right. When you take a closer look at it you can see, that the edges of this spot are very diffuse. This is a clear sign for damage, possibly by hatching or short while after hatching of the pupa.

| | | Re: 2-spot Ladybird with 3 spots I did notice some diffusion Klaas, but the shape convinced me it must be a spot! It is strange that the damage shoud be so spot-like but I guess you are correct.
