Hi John,
There are various ways that your finds might be recorded, including the use of specific database type software.
However, if the lists would be solely for your own use, I'd suggest that a basic spreadsheet would easily suffice.
I record my finds on a Microsoft Excel spreadsheet, which I created to suit my own needs, but keeping within the parameters of what is needed for County records, and generally in accordance with BMS guidelines. (I submit my records via the North West Fungus Group).

Basically, my spreadsheet comprises of the following columns: -
Latin Name - Self explanitory requirement.
Common Name - The Accepted English name where applicable,

Site - The name of the site where the fungus was found.
County - The county in which the site is located.
Vice County - The Watsonian Vice County number for the site.
Grid Ref - I record in the format SD74053635
Association - eg. angiospermae, gymnospermae, poaceae, pteridopsida, etc. etc.
Substrate - The specific substrate in/on which the fungus was growing.
Ecosystem - eg. broad-leaved semi-natural, coniferous plantation, etc. etc.
Fruitbody - The number of that particular species found.
Date - Self explanitory requirement.
Collector - The name of the person making the find.
Determiner - The name of the person making the identification.
Herbarium - Details of any material dried & retained, or photographs taken etc. etc.
Literature - Details of any literature used to facilitate identification.
Notes - Any additional info not covered by the specific headings above.
You might not need to go to these lengths, but the more information you record, the easier it is to return to that record when cross referencing is needed at some future time.
