I've a 120-400mm Sigma that lives in my AW 200 backpack but when in use I use both the camera strap and the one on the lens and have them over my shoulder.
Note one has to be longer than the other but it's worth hanging the camera up until you get them both the same length (when on your shoulder).
That way the weight is taken by both straps equally and so less stress on the kit.
